THIS IS THE MAIN PAGE OF "CASE MANAGEMENT" IN "TRAINING AND SUPPORT SERVICES"
Each consumer works with his/her case manager to develop a set of rehabilitation goals. The case manager coordinates an evaluation and training program which will facilitate movement toward these goals.
In the initial stages of training, a consumer has an
opportunity to network and build a team back home, identify community resources,
and initiate job development activities. The consumer's team includes the HKNC rehabilitation staff,
the
regional
representative from his/her home state; his/her
state vocational rehabilitation counselor, and other service
providers. This team works together to assist each consumer in securing
employment, locating a suitable home, and identifying and obtaining
community supports and services.
A case
manager may also provide vocational counseling with a focus on programming
issues and vocational exploration.
